I sign up my employees?
Just
4 Simple Steps
Step
1: Select the plan and print the below Applications and
distribute to your employees to complete.
Step
2: Print the Group Agreement
and complete.
Step
3: Print off Provider Lists
and Schedule of Plans and give to your employees.

you have a group! (at least 3 must

4: Mail in
the following:

applications (from Step 1 above)

Include
check for first month's membership fees made out to Careington

it in.

Your dental
plan will begin the 1st of the following month! Please note on a
cover letter to back date the group if applications are sent in
after the first of the month.
